Publik/masyarakat dapat menyampaikan pengaduan dengan memenuhi persyaratan sebagai berikut:
- Warga Negara Indonesia (WNI);
- Mengisi formulir pengaduan masyarakat [Unduh];
- Melampirkan fotokopi identitas diri yang masih berlaku (KTP/SIM/ID Card);
- Dapat menjelaskan siapa, apa, bilamana, dimana dan bagaimana kejadian yang dilaporkan (kronologis aduan);
- Melampirkan bukti awal aduan, seperti: fotokopi dokumen, foto atau barang lain yang dapat memperkuat uraian aduan yang disampaikan.
- Dokumen yang wajib dilampirkan adalah sebagai berikut:
- scan formulir (halaman 1 dan 2) yang sudah diisi dan ditandatangani (sesuai panduan pengisian);
- scan/fotocopy identitas diri;
- bukti awal aduan.
- Pengaduan tersebut beserta lampiran, silahkan dikirimkan kepada BPK Perwakilan Provinsi Jawa Timur dengan cara:
- Datang ke Kantor BPK Perwakilan Provinsi Jawa Timur dan diserahkan kepada petugas secara langsung atau dimasukkan ke dalam kotak Pengaduan Masyarakatyang tersedia
